Ways to Give

Gifts of Gratitude

Gratitude is often part of the healing process. Many patients are grateful for our clinicians and the care they provide. You have the option of giving online with a credit card. If you prefer to mail your donation, please make your check payable to Mosaic Health System and note the fund name or area you wish for your gift to be directed. Please mail your gift to:

If you wish to give securely over the phone, please call (816) 271-7296 and provide how you’d like your gift directed and if you’d like to make it in honor of a loved one or Mosaic caregiver.

Memorials and Tributes

A memorial or tribute gift is a meaningful way to make a difference by recognizing a loved one, a special occasion or honor an individual who made a difference in your life. Upon receipt, the donor(s) will receive a gift acknowledgement, and we will notify the designee/family of your thoughtful gift with a list of all donors should they wish to personally acknowledge each gift. We do not disclose the amount, only that a gift was made to the named fund.

Memorial Gift Sample Language

In lieu of flowers, the family asks that donations be made to Mosaic Life Care for the benefit of the health system. Please make checks payable to Mosaic Health System and send them to:

Planned Giving + Other Ways to Give

Legacy giving does not require a large income or vast estate. A planned gift only requires a thoughtful intention to make a difference for patients by remembering Mosaic in your will or estate plans. 

Whether you give to Mosaic Life Care through a transfer of assets from your estate or trust, a gift of appreciated stock, selling a business or a gift of grain, every gift helps to ensure access to quality care across our rural communities regardless of one’s ability to pay. It can be as simple as changing a beneficiary designation or making an amendment in your retirement plan that honors your wishes.

careGIVER Giving

Mosaic comprised of nearly 5,000 physicians, nurses and staff members who are dedicating their professional lives to make a difference – one patient, one family at a time. Many of those caregivers generously donate to funds that can directly impact our patients and fellow caregivers in times of need. If you’re a caregiver of Mosaic and would like to make a gift through payroll deduction, please visit Shared Services.

Your Gifts Make a Difference

We want giving to be a joyful experience for you and yours. That is why donors like you can choose exactly how your gift is put to work. We have needs that span nearly every area of care across our health system. When making your donation, please identify what area you’d like your gift to go to. Examples include cancer care, heart care, patient experience, rural medicine scholarships, area of greatest need, and more.
