Methodology

The community health needs assessment was facilitated by Crowe LLP (“Crowe”).

Crowe is one of the largest public accounting, consulting, and technology firms in the United States.

Crowe has significant healthcare experience including providing services to hundreds of large healthcare organizations across the country.

Based on current literature and other guidance from the Treasury and the IRS, the following steps were conducted as part of Mosaic’s CHNA:

Community input was obtained through key stakeholder interviews of community leaders representing twenty-four organizations.

Three focus groups were conducted within Mosaic Life Care.

  • Senior Leadership
  • Physicians
  • Nurses/Social Workers/Care Managers

Community input was also obtained through a community-wide survey for the general public. More than 700 surveys were completed.

Primary Health Needs Identified:

  1. Mental Health/Substance Abuse
  2. Access to and Cost of Health Care
  3. Obesity – Adult and Child

To fully meet the needs specified by the CHNA research results, Mosaic Life Care will implement a three-year action plan.
