Maryville Community Health Needs Assessment

On April 1, 2019, Mosaic Life Care and SSM Health finalized the purchase agreement to transfer ownership of SSM Health St. Francis Hospital – Maryville to Mosaic Life Care. Mosaic assumed operations on April 1, 2019. The below CHNAs were conducted by SSM Health to fulfill SSM Health’s CHNA reporting requirements. A new CHNA for Mosaic Medical Center – Maryville will be conducted within the timeframe required by Section 501(r)(3) requirements with respect to being an acquired hospital facility. This due date is June 30, 2021.
