Our patients

LTACH’s patients are aged 18 or older and vary in condition. From an elderly patient who is suffering with pre–existing conditions to someone who is technology–dependent – our patients are referred here for continuation of acute medical care because the severity and complexity of their condition precludes them from being accepted into a rehabilitation hospital or a skilled nursing facility.

Our patients’ diagnoses

We provide care for patients whose diagnoses include, but are not limited to:

• Respiratory failure requiring ventilator management
• Cardiopulmonary or cardiovascular diseases
• Respiratory disorders
• Post–surgical complications
• Wound care
• Infectious diseases
• Neurological conditions
• Musculoskeletal diseases
• Cancer–related conditions
• Congestive heart failure
• Stroke or a cerebral vascular accident
• Complex or orthopedic conditions
• Multi–symptom disorders
• Nutrition therapy
