Different bills you might receive

Depending on your type of treatment, you may receive more than one bill. For example, if you had your appendix removed, you may receive a bill from the hospital for the operating room and surgical supplies. You may also receive a bill from the surgeon who performed the surgery and the doctor who supervised the anesthesia.

Hospital charges do not include physicians' fees. You will be billed separately for the professional services of the emergency room physicians, your admitting physician and other physicians involved in consultation. For example, radiologists will bill you for interpretation of x-ray procedures; anesthesiologists will bill you for the administration of anesthesia during surgery; pathologists will bill you for their services. You are responsible for providing insurance information to those other providers. Sitters must also be paid separately.

Forms of payment

You can pay your bills with cash, VISA, MasterCard, Discover, debit cards, Paypal, personal check or money orders. Payments made after the date on the statement will appear on the next month's statement. If you are unable to pay the full amount, we have payment options that may help. Patients may call Patient Billing at (884) 261-7266. We want to work with each patient to meet his or her financial responsibilities. If this isn't successful, past due amounts will be forwarded to an agency to continue collection efforts.

Insurance information

If you have health insurance, the amount you will pay for care will depend on your insurance plan. Your insurance plan's benefits package will determine how much you pay and how much your health insurance plan will pay.

You will also want to make sure Mosaic Life Care is in-network for your insurance coverage. If your insurance company says Mosaic Life Care is not in-network for your plan, you will want to find out how your out-of-pocket costs will be affected if you use an out-of-network doctor. The price you pay is usually lower for in-network doctors.Understanding your EOB
