Inpatient Care Management - Care Transitions
Care Managers who are registered nurses and Medical Social Workers work with you, your family and the healthcare team to coordinate the care you receive during your stay. The primary focus of Care Management is to support high quality, cost effective care and identify options for any continuing care needs you may have upon discharge. We work with patients, families, and their physicians to offer options and arrange for:
  • Skilled nursing or extended care facility transfers
  • Home health or hospice
  • In-home support services
  • Medication assistance programs
  • Medical equipment 
  • Transportation
  • Community resources
  • Assistance with insurance benefits and coverage
  • Financial assistance programs

For patients with complex needs or who experience an extended stay, the Care Management team may also provide:

  • Coordination of family conferences with the physician
  • Support group information and education
  • Assessment and intervention for cases of abuse and neglect
  • Assistance and support in understanding their condition and treatment plan

We recognize your right to choice in your treatment plan and options of providers. All service arrangements are at the direction of the patient or a representative with guidance from the physician.
